Unlike traditional clinical educators focusing strictly on one-on-one episodic appointments, this role leverages electronic health records (EHR) and disease registries to proactively track clinical gaps, manage rising-risk diabetic cohorts, and coordinate systemic preventative interventions. The primary goals are to optimize glycemic outcomes, close quality metrics gaps, and reduce preventable emergency department utilization and hospital readmissions.
