- JOB REQUIREMENTS
- Education
- High school diploma or GED required. Must be licensed with the Arkansas State Board of Pharmacy as a pharmacy technician or have an LPN license before the onset of duties. National certification for pharmacy technicians is preferred , acceptance into a professional post graduate program (medicine or pharmacy), or a minimum of 2 years experience.

- Experience
- Excellent verbal communication skills, ability to work in a team environment and ability to work in stressful environments is essential. The ability to effectively prioritize and manage multiple tasks is also desired. Pharmacy/healthcare experience with direct patient care roles is preferred.

- Normal hospital environment. Close eye work. Hearing within normal range. Operates computer, typewriter, calculator. Finger dexterity. Eye-hand coordination. Frequent sitting, standing and walking. May lift and carry up to 35 lbs.

- JOB SUMMARY

The Transitions of Care (TOC) Pharmacy Technician plays a vital role in supporting safe and effective medication use during a patient’s transition between healthcare settings—such as admission, discharge, or transfer between care facilities. Under the supervision of a licensed pharmacist, the TOC Pharmacy Technician is responsible for gathering accurate medication histories, reconciling medications, coordinating communication between healthcare providers, and facilitating medication dispensing and delivery post-discharge. Technicians will play an important role in promoting the Meds to Beds program as a continuity of care service and facilitate medication dispensing activities in the pharmacy.
This role requires strong communication, attention to detail, and organizational skills to help reduce hospital readmissions and improve patient outcomes during critical points of care transition.
